Winger Stella Wafula starts on the blind wing swapping with Christabel Lindo who moves to the open side to see Diana Awino drop to the bench.
Centre Sheila Chajira is fit to start as she comes in for Celestine Masinde who drops to the bench.
Prop Evelyne Kalemera replaces Imgone Hooper(bench) while Immaculate Owiro will pair Enid Ouma on the second row to see Bernedette Olesia miss out on matchday 23. Flanker Naomi Amuguni is the only change at the back row.
Kenya kicked off the qualifiers with a 35-5 win over Madagascar while Uganda lost 89-5 to hosts South Africa.
This tournament will determine the first regional qualifier for Women's Rugby World Cup 2021 in New Zealand. The winner of the round-robin will qualify directly while the loser will head to a pre-repechage playoff with an opponent from South America.
Kenya and Uganda met in July in the Elgon Cup with Lionesses winning 35-5 in Kampala and 44-14 in Kisumu to complete a 79-19 win on aggregate.
South Africa will play Madagascar at 4 pm.
